What the work is like

Treating patients in OPDs, dispensaries and hospitals run by the Central Government, Indian Railways, NDMC and MCD. Also public health work such as sanitation, family planning and health checks, and handling emergencies like railway accidents.

A day in the work
You reach your hospital or dispensary in the morning and see a long OPD queue of patients, many of them government employees and their families. A railway doctor may also check a station health unit, inspect food vendors or look at sanitation in railway colonies. You write prescriptions, order tests, fill registers and sometimes join an emergency call. In the afternoon there are reports and returns to complete, and your team of nurses and staff needs guidance.

Professional qualificationRun by National Medical Commission (NMC) and State Medical Councils. You can start after: Class 12 with Physics, Chemistry, Biology and English, plus NEET-UG. Minimum time: About 5.5 years after Class 12 for MBBS with internship, then the CMS exam.

The full route: exams, training, registration

  1. Step 1 · Class 11–12Take Physics, Chemistry, Biology and English. Class 12 marks in PCB and a good NEET score both matter, so build your Biology basics early.
  2. Step 2 · After Class 12Write NEET-UG and join MBBS through counselling. There is no upper age limit for NEET and no cap on attempts, and the score is valid for that year's admission only (reported). (reported — check with the official body)
  3. Step 3 · About 5.5 yearsComplete MBBS, including the 1-year internship, and register with the State Medical Council or NMC. UPSC accepts final-year students for the exam. But you must have passed the written and practical parts of the final MBBS exam to be appointed (reported). (reported — check with the official body)
  4. Step 4 · CMS written exam (yearly)In 2026 UPSC issued the notice on 11 March, took applications till 31 March and held the exam on 2 August. There are two papers of 250 marks each, 2 hours each. Paper I is General Medicine and Paediatrics. Paper II is Surgery, Gynaecology & Obstetrics, and Preventive & Social Medicine.
  5. Step 5 · Interview (Personality Test)Candidates who qualify the written exam are called for an interview of 100 marks (reported). In 2026 the written result came on 1 September. UPSC then asked qualified candidates to upload documents, and the interview schedule was published on 25 September. Your result is provisional until your documents are checked. (reported — check with the official body)
  6. Step 6 · Medical fitness and allotmentYou must meet the physical and medical standards given in the exam rules. Your service (Central Health Service, Railways, NDMC or MCD) is then allotted from your final merit rank and your preferences (reported). (reported — check with the official body)
  7. Step 7 · Training and probationRailway ADMOs join on 1 year of probation and begin with a Foundation Course. Central Health Service and NDMC doctors have 2 years of probation, and it can be extended. You are confirmed after satisfactory service (reported from a copy of the 2026 notice). What people earn

Official pay

Pay Level 10 of the 7th CPC Pay Matrix, ₹56,100–₹1,77,500, plus Non-Practising Allowance (NPA) and other allowances. This applies to Central Health Service, Railways ADMO and NDMC posts, and the MCD GDMO Grade-II post starts at the same level (2026 notice) (reported, read from a copy of the UPSC notice).

Pay starts at a fixed government level and rises with yearly increments and promotions, and posting city changes allowances like house rent. You earn less than a busy private specialist, but pay is steady and comes with job security.

Myth and fact

Myth: A CMS doctor can work in a government hospital and also run a private clinic in the evening.
Fact: In the Central Health Service, private practice of any kind, including consultant and lab work, is prohibited. In return you get Non-Practising Allowance (NPA) and a steady salary.

For girls and students with disabilities

The 2026 notice says women candidates are encouraged to apply, and women pay no exam fee. It also reserved 34 Central Health Service vacancies and 18 Railway ADMO vacancies for persons with benchmark disabilities, in listed categories only (for example, the Railway ADMO posts were for locomotor disability). Candidates must still meet the medical standards in the rules, and age relaxation for PwBD is usually 10 years (reported).
