Is this for me?

People in this work enjoy
  • Helping people close to home and seeing the same families get healthier over time
  • Leading a small team and being the first point of care for a whole village
  • A mix of treating patients, prevention work and talking to the community
It may not suit you if you dislike
  • Posting is often in a village, away from city life, and you may need to stay near the centre
  • Many posts are on contract, and pay or incentive rules can change or be disputed
  • You work with limited equipment and few senior doctors close by, so you must be careful and know when to refer
Class 11–12 subjects
Most Community Health Officers (CHOs) start with Class 12 science: Physics, Chemistry, Biology and English, for B.Sc Nursing (usual rule; check each college and state, reported). Some start from GNM or BAMS and add a 6-month community health certificate later.
First degree
Most common route: B.Sc Nursing (4 years) with a community health certificate built into the course. Other routes: GNM or BAMS plus a 6-month bridge certificate, or a stand-alone B.Sc Community Health, which very few colleges run.
Entrance exams
There is no single national exam. Each state's National Health Mission (NHM) issues its own notice. For example, the NHM Uttar Pradesh notice (about 2024) selected CHOs through a computer-based test and then document verification.
Licence or registration
Valid registration with your State Nursing Council (for nurses) or the relevant AYUSH council (for BAMS) is asked for in state notices (reported). Check each notice.

What the work is like

Lead the team at an Ayushman Arogya Mandir (the new name for Health and Wellness Centres). Treat common illnesses using protocols, screen for blood pressure and diabetes, care for mothers and children, and run health talks. Refer serious cases and keep records.

A day in the work
You open the sub-centre in the morning and see patients with fever, cough, high blood pressure or sugar problems. You give medicines from the free list and refer anyone who needs a bigger hospital. After that you check pregnant women and young children, and plan the day with the ANM and ASHA workers. In the afternoon you may walk to a nearby hamlet for a health talk or a screening camp. Then you update records on a tablet or in registers and send your reports.

The path, step by step

  1. Step 1 · Class 11–12Take Physics, Chemistry, Biology and English. These are the usual entry subjects for B.Sc Nursing (reported). Check the college's own rule.
  2. Step 2 · After Class 12 · 4 yearsJoin B.Sc Nursing at a recognised college. Ask whether the course includes the integrated community health certificate. Many state notices now ask for it, and some say a normal nursing degree without it is not enough (reported). (reported — check with the official body)
  3. Step 3 · After the degreeRegister with your State Nursing Council. State notices usually ask for valid registration (reported). (reported — check with the official body)
  4. Step 4 · State NHM noticeApply online when your state's NHM announces CHO posts. Each state sets its own age limit, qualification and domicile rules. In the NHM UP notice (about 2024), shortlisting was through a computer-based test.
  5. Step 5 · Document verification and postingAfter the test, your marks and papers are checked. You are then posted to a sub-centre Ayushman Arogya Mandir in a district, often in a village.
  6. Step 6 · Work on contractThe NHM UP notice offered contractual posts, not regular government jobs. Contracts are usually renewed each year (reported). Training on the job continues.
  7. OptionalIf you are a GNM nurse or BAMS doctor, take the 6-month IGNOU Certificate in Community Health to become eligible. Your state must sponsor you. You can also study further for public health, nursing or teaching roles.

Ways in

  • B.Sc Nursing with integrated community health certificateStarts after: Class 12 science (PCB) 4-year B.Sc Nursing, then State Nursing Council registration, then state NHM recruitment. This is the route most state notices now ask for (reported).
  • GNM or B.Sc Nursing, then a bridge certificateStarts after: Nursing qualification IGNOU's 6-month Certificate in Community Health. Admission is only for candidates sponsored by their state government. Nurses already working for the state or NHM can be sent on it.
  • BAMS, then a bridge certificateStarts after: BAMS degree The same IGNOU bridge certificate is open to Ayurveda graduates. States hire 'AYUSH CHOs' under their own rules (reported). Some states restrict private practice for them (reported).
  • B.Sc Community HealthStarts after: Class 12 science The Cabinet approved this 3½-year degree in 2013, but uptake was slow. Very few colleges run it, so check that recruiters accept it before joining.

Your real chances

There are many posts, but they are spread across states and recruited in rounds. The NHM Uttar Pradesh notice (about 2024) alone advertised more than 7,400 contractual CHO vacancies. As of 30 November 2024, the Health Ministry's portal showed 1,16,978 CHOs in position across India, and 1,81,873 Ayushman Arogya Mandirs were running by 30 November 2025. I could not find official applicant numbers.

Good to knowRules and pay differ by state, and CHO posts are contract jobs, not regular government service; the IGNOU bridge route needs state sponsorship and a 3-year sub-centre bond applies.

What people earn

Official pay

CHOs are paid as contract staff, not on a government pay level. The NHM Uttar Pradesh notice (about 2024) gave Rs 25,000 a month honorarium plus up to Rs 10,000 a month performance-based incentive. Other states set their own amounts, and some later notices report a higher incentive of up to Rs 15,000 (reported).

Earnings depend mostly on your state and how much of the incentive you earn, and they tend to move slowly with experience because most posts are on contract; later roles in public health, nursing education or hospitals can pay differently.

Myth and fact

Myth: A Community Health Officer is a government doctor with a permanent job.
Fact: A CHO is a mid-level health provider, usually from nursing, GNM or BAMS, not an MBBS doctor. Many CHO posts are contractual under the National Health Mission, as in the NHM UP notice.

For girls and students with disabilities

Each state's NHM notice sets its own reservation, age relaxation and any rules for women or persons with disabilities, so read the notice for your state. The work involves travel to villages and a lot of standing and walking, so ask about physical needs before applying.

Try it now

  • Class 7–9Visit your nearest Ayushman Arogya Mandir or sub-centre with a parent. Ask the staff what a typical day looks like and what services are free.
  • Class 10–12If you are 15 or older, register on MY Bharat and look for local health camp or awareness volunteering. Keep Biology strong, since nursing entry usually needs PCB.Open link ↗
  • College or afterBrowse free SWAYAM courses on public health, nutrition or community health. Then read your own state's latest NHM CHO notice to see its exact eligibility.Open link ↗

Good to know

  • There is no one national CHO exam or notice. Rules differ from state to state, so always read your own state's NHM notice.
  • Many CHO posts are on contract under the National Health Mission, not regular government service. Pay, renewals and incentives have caused disputes in some states. In Punjab, for example, CHOs went on strike over pay (reported).
  • The Health Ministry and the National Commission for Allied and Healthcare Professions (NCAHP) are bringing in new standard curricula. The PIB release says they will be mandatory from 2026. Ask a college which curriculum it follows.
  • If you want to treat patients as a doctor, this is not that route. For that, look at MBBS, BAMS or other doctor courses.
