Is this for me?

People in this work enjoy
  • Seeing a patient who could not walk take their first steps again
  • Working with people one-to-one, using your hands and your explanations
  • Variety: children, sportspeople, older adults, and patients after surgery or stroke
It may not suit you if you dislike
  • You are on your feet a lot, and lifting or supporting patients is physically tiring
  • Recovery can be slow, and some patients do not improve, which can be emotionally hard
  • Entry rules are changing (NEET from 2027-28) and the first years of work can pay little
Class 11–12 subjects
Class 12 with Physics, Chemistry and Biology (or Biotechnology) and a pass in English. Minimum marks in PCB are reported as 50%, or 40% for SC, ST, OBC-NCL and PwBD candidates (reported). Minimum age is reported as 17 years at admission (reported).
First degree
BPT (Bachelor of Physiotherapy). Older courses ran 4.5 years including internship. The newer national curriculum is reported as 4 years of study plus a 1-year internship (reported). Optional MPT (Master of Physiotherapy), 2 years.
Entrance exams
For 2026-27, NEET UG was not compulsory for BPT. The National Commission for Allied and Healthcare Professions (NCAHP) deferred that rule to 2027-28, so colleges used Class 12 PCB marks or their own entrance test. For 2027-28 and later, appearing in NEET UG is to be mandatory (reported). Some central institutes, such as SVNIRTAR Cuttack, ran their own Common Entrance Test (CET) in 2026. Check the latest rule before you apply.
Licence or registration
Registration on the State Register kept by your State Allied and Healthcare Council, under the NCAHP Act, 2021. The certificate is valid for 5 years and then must be renewed. New 2026 registration rules are reported to add Continuing Professional Development, 15 hours a year (reported).

What the work is like

Checking how a patient moves, finding the cause of pain or weakness, and planning treatment. Treatment includes exercises, hands-on techniques and machines such as heat, ultrasound or electrical stimulation. Working with patients after fractures, joint replacements, strokes, heart or lung illness, or with children who have movement problems. Teaching patients and families what to do at home. Working with doctors, nurses and occupational therapists.

A day in the work
You start the morning by reading the notes of the patients you will see. A man who had a knee replacement last week practises bending his knee and walking with a frame, while you check his pain and progress. Next you work with a woman recovering from a stroke, helping her stand and use her weak arm, and you teach her daughter the exercises to do at home. In the afternoon you may be in the ICU helping a patient breathe better, or treating a child at a clinic. You end the day writing notes and telling the doctor how each patient is improving.

Professional qualificationRun by National Commission for Allied and Healthcare Professions (NCAHP) and State Allied and Healthcare Councils. You can start after: Class 12 with Physics, Chemistry and Biology. Minimum time: About 4.5 to 5 years after Class 12 (BPT plus internship).

The full route: exams, training, registration

  1. Step 1 · Class 11–12Take Physics, Chemistry and Biology, and pass English. Aim for at least 50% in PCB, or 40% if you are in a reserved category or have a benchmark disability (reported). Maths-only (PCM) students cannot join BPT.
  2. Step 2 · Class 12Check the admission route for your year. For 2026-27, seats were filled on Class 12 PCB marks or a college or institute entrance test. NCAHP's letter to the UGC (2 March 2026) deferred the NEET UG requirement to 2027-28. If you join in 2027 or later, plan to appear in NEET UG. Colleges can still have their own selection steps, so read each college's notice. (reported — check with the official body)
  3. Step 3 · 4 yearsBPT at a college recognised under the NCAHP framework. You study anatomy, physiology, exercise science, and the physiotherapy of bones and joints, nerves, heart and lungs. You also do supervised clinical work with patients. The newer national curriculum is described as 4 years of study (reported). (reported — check with the official body)
  4. Step 4 · Internship (6 months to 1 year)A compulsory full-time rotating internship in hospital departments. Under the older pattern the whole course, including internship, was 4.5 years. The newer curriculum is reported to make the internship a full year (reported). You usually need to finish it before you can register. (reported — check with the official body)
  5. Step 5Register with the State Allied and Healthcare Council of the state where you live. The NCAHP Act says the registration certificate is valid for 5 years. Renew it before it lapses. Some states are still setting up their councils, so check the process in your state.
  6. OptionalMPT (2 years) in a speciality such as musculoskeletal, neurology, paediatrics, cardiopulmonary or sports. At SVNIRTAR Cuttack, MPT entry is through its own PGET. You can also study further to teach or do research.
Recently changed, or still changing
  • NEET UG for BPT was planned from 2026-27 but was deferred by NCAHP to 2027-28 (reported). Check the latest notice when you apply.
  • NCAHP notified Registration Regulations in 2026 that are reported to cover physiotherapists, with 5-year validity and ongoing learning hours (reported). State councils are still being set up in some states, so procedures differ.

What people earn

Earnings are modest for freshers, usually rising with experience, a speciality, a good hospital or sports team job, or a busy private clinic, and they differ a lot between big cities and smaller towns.

Myth and fact

Myth: Physiotherapy is just massage and a few exercises, so it is an easy course.
Fact: Physiotherapists study anatomy, physiology and clinical medicine, then assess patients and plan treatment across bones, nerves, heart and lungs. It is a clinical rehabilitation profession with a degree, supervised hospital internship and a registration requirement.

For girls and students with disabilities

At national institutes such as SVNIRTAR, 5% of seats are reserved for candidates with benchmark disability (40% or more), along with SC, ST, OBC and EWS reservation. Minimum Class 12 marks are reported as 40% for reserved categories and PwBD (reported). Physiotherapy has no separate rule for women, and girls study and work in it in large numbers. You can ask the college about physical fitness requirements.

Try it now

  • Class 7–9Learn how muscles, bones and joints work from your Science book. Then try noticing your own posture while you sit and study, and do simple stretch breaks.Open link ↗
  • Class 10–12Ask a physiotherapist you know, or your school's physical education teacher, how they got into the work. If a hospital or rehabilitation centre allows it, ask to watch a department for a day with your parent's permission. Keep Biology strong, because PCB marks decide your eligibility.
  • College or afterVolunteer at a government hospital's physiotherapy department, a rehabilitation centre or a local sports club. Watch how therapists talk to patients and track their progress.

Good to know

  • Physiotherapy comes under the NCAHP Act, 2021, not the National Medical Commission. A physiotherapist is not an MBBS doctor and does not prescribe medicines.
  • Before you join a private college, check that the course is recognised under the NCAHP framework and that it has a hospital where you will get enough patients to treat. A pretty campus matters less.
  • Rules about NEET, course length and registration changed in 2026. Many websites are out of date, so check NCAHP, your state council or the college's own notice.
  • If you are not selected for BPT, occupational therapy, nursing and nutrition are neighbouring health careers that also need Class 12 Science.
